The Health Journal is an optional resource for program participants of the StrongPeople® programs. It includes monthly SMART goal setting pages, weekly meal and physical activity planning pages, daily tracking pages and weekly and monthly progress tracking pages. It is set up chronologically like a planner, and includes 6 months of planning pages.
This booklet is for use in StrongPeople® programs. Any unauthorized copying, alteration, distribution, transmission, display, or other use of StrongPeople® materials is prohibited.
